Many health insurance plans require pre-authorization
from referring primary care (or family) physicians
before their patients can receive medical services
from one of SERA's specialists. The patient is
responsible for providing the authorization to
SERA before their first visit.

Patient Forms and Information Packets
Each Southeastern Retina Associates office uses
New Patient Information Forms designed especially
for their location and patients. Your patients
will need to print and fill out the proper forms
and bring them to their first visit, along with
their insurance cards. The forms are available
on the SERA Website, arranged by physician's names.
